Once you have received your FREE PET EXAM Rebate Certificate, make an appointment with the vet of your choice. Whether they are around the block, around the corner, or across the state, you may use any veterinarian as long as they are a licensed Veterinarian.

After
your pet’s check-up with a veterinarian of your choice,
pay the bill and take your pet home. |

When you get home, simply attach a copy of the bill
